Do you accept health insurance?

• We accept both private pay and take most health insurance plans. We are in network with the following insurance companies: Aetna, Horizon Blue Cross Blue Shield, Medicare (only covers for diabetes and renal disease) and United Healthcare insurance plans. If you have coverage through any of these providers, we can assist to help you understand your benefits and coverage for nutritional counseling. Please note that coverage can vary based on your specific plan, so we recommend first contacting your insurance provider directly for verification of your benefits. If you have a different insurance provider that we are not in network with but you have out of network benefits, we can provide a superbill for submission. If you do not have health insurance, we offer self-pay packages that are competatively priced.
